Students or Staff:
Please complete this short check each morning before leaving for school. Students or staff who are sick should not attend school in-person.
Please check for the following Symptoms:
- Temperature 100.4 degrees Fahrenheit or higher
- Sore throat
- New uncontrolled cough that causes difficulty breathing (for students with chronic allergic/asthmatic cough, a change in their cough from baseline)
- Diarrhea, vomiting, or abdominal pain
- New onset of severe headache, especially with a fever
Please also check for any Exposure
Had close contact (within 6 feet of an infected person for at least 15 minutes) with a person infected with COVID-19*, or
Live in or traveled to a county where the SD-DOH is reporting substantial spread of COVID-19 cases
